To help things run smoothly, we ask that you arrive about 10 minutes early to complete your intake forms. If you can, please bring a few helpful details: your health insurance card and booklet, a list of any medications, allergies, or medical conditions, and your family doctor’s name and phone number.

After a full exam, we’ll walk you through a personalized care plan designed around your needs, comfort, and schedule. Whether it’s a routine checkup or something more involved, you’ll leave with a clear understanding of your options, and a plan that makes you feel confident and cared for.
Payment options that work for you
We keep things easy when it comes to payment. We accept most traditional insurance plans and are happy to help you file for reimbursement. Payment is due at the time of your visit, and we offer flexible payment plan options if you need to plan ahead—please see our Financial Coordinator for details. Cash, card, or check, all are welcome.
